* You will have responsibility for the day-to-day delivery of the service to individual clients and their families and carers via a range of evidence-based interventions.
* You will be required to deliver specific interventions in line with the treatment pathway.
* You will develop your assessment skills by working with experienced clinicians
* Generate options including modification or alternatives to planned intervention to meet clinical need as appropriate.
* Agree course of action with those responsible for performing the interventions including supporting junior staff.
* You may be required to provide supervision for Nursing Associates and support workers in the team.
* You will carry out Assessments and plan care delivery for individual service users within the framework of the current Care Programme Approach
* Enable service users to choose and take part in activities that are meaningful to them. These may include self care, education, recreational and work activities.
* Participate as part of the multi-disciplinary team implementing, evaluating and reviewing all aspects of the service users' care
* Advise, inform and support on the physical health and wellbeing of individuals (services users, their family, their significant other) in a holistic way.

The post is a substantive position and would be a developing role for a newly qualified band 5 precepted This is a county wide service across Coventry and Warwickshire. We have vacancies available within Coventry, North Warwickshire and South Warwickshire, the successful candidate will be able to be based predominately within one area, but will need to be able to work across locality covering all three areas when needed. A full UK driving licence and access to your own transport is essential in this role.
