The Vacancy
This role is based within Platfform’s Supported Housing scheme within the Vale of Glamorgan. The scheme provides tenancy related support to persons with complex and/or enduring mental health needs as evident either through their support history, because they are under the care of secondary mental health services, or both.
The successful candidate will provide a range of social, emotional and practical support to people with mental health and associated problems impacting their ability to maintain a stable tenancy, through semi-independent living. The support will be expected to be within commissioner’s guidelines i.e. the Supporting People Framework, and in line with Platfform’s value base.
The role holder will be expected to provide support and mentoring to Support Workers in the development of individual support plans (ISPs) and risk assessments (RAs), as well as take the lead during crisis support.
Compliance with and maintenance of all necessary administrative and reporting systems will be required.
