The Home First Single Point of Access Team is looking to recruit a permanent social worker to work a shift pattern over 7 days a week. We work with individuals aged eighteen plus, which means our work is varied and covers individuals with a range of different needs.
The right candidate will have a social work degree and be registered with Social Work England. They will be a strong team player with a proactive approach to working as part of a busy Social Work Team. These positions are open to experienced workers and Newly Qualified Social Workers, and we offer an ASYE year.
The successful candidate will be highly motivated and willing to undertake Care Act Assessments, support/care planning, and commission services with person-centred outcomes. They will provide social work supervision and have experience in social work management. The candidate must be able to travel independently throughout North Lincolnshire, as we cover the entire area. Knowledge of adult social care legislation and frameworks is essential.
You will need to demonstrate problem-solving abilities, good negotiation skills, effective communication, organizational, and IT skills. Flexibility, resilience, and dedication to teamwork are also required.
