Job overview
An exciting opportunity has arisen within the Shrewsbury & Telford Hospital for a Heart Assessment Team Staff Nurse (Band 5) managing heart failure and cardiac rehabilitation patients. This position will be based at the Royal Shrewsbury Hospital. Within this role, there may be the need to work across site.
The successful applicant will be a qualified NMC registered nurse with proven post-registration experience in cardiology.
Main duties of the job
Working as an autonomous practitioner, the post holder will work as part of a multi-disciplinary team, meeting the needs of patients referred to the Heart Assessment Team. Within the role, the post holder will provide ongoing support and lifestyle advice to patients with Coronary Heart Disease/Heart Failure on an inpatient/outpatient basis through one-to-one counselling and group sessions. The post holder will undertake patient assessments prior to the commencement of cardiac rehabilitation exercise programmes and supervise planned exercise sessions.

* RGN
* Coronary Care Course or equivalent
Experience and Knowledge
* Awareness of relevant/current professional issues
* Previous experience of working in the field of cardiology
Skills and Ability
* Ability to work and communicate effectively within a MDT

* Microsoft Office PC and SEMA PAS skills
