Salary: £11.39 per hour (admin rate), £15.18 (1-1 session) plus mileage paid

As a Diabetes Practitioner you will be coaching service users on the Type 2 Diabetes Remission program, delivering 1-1 coaching via face to face sessions. A Key focus will be to encourage and help service users to make behavioural changes, set healthy lifestyle goals and complete the programme, assisting them to overcome potential barriers that they are facing.

Every day you will...
Work closely with service users throughout the programme delivering a first class service and knowledge
Conduct 1-1 coaching sessions to service users across your allocated areas, often taking measurements such as weight, blood glucose and blood pressure.
Maintain data monitoring records of all service users and ensure that this is handled with the greatest care
Support the service user engagement strategy to ensure adherence to the programme and minimise drop-outs

To thrive in this role, you must have...
Minimum of undergrad Degree in Nutrition or Health Related Science (Sports/Exercise Science/Health Psychology OR experience in personal training with a level 2/3 PT qualification
A full UK driving license and access to your own vehicle
Availability to work a minimum of 3 days per week, with some flexibility to cover occasional evenings and weekends
