The post holder will be an active member of the Fracture Liaison Service, responsible and accountable for assessing, implementing, and evaluating clinical care and outputs for patients to meet their needs in a safe caring environment that promotes dignity, privacy, and respect by using evidence-based practice to underpin professional judgment and clinical reasoning.
The post holder will work in collaboration with the Medical staff, Nursing staff, and Primary Care colleagues.
The post holder must have the ability to communicate effectively and to a high level and maintain high standards of care at all times. Delivering a patient-focused service, the post holder will act as the patient's advocate at all times.
They will be required to input data and clinical outputs into the National Database and have oversight of predefined KPIs.
They will be flexible and adaptable and work towards the ultimate aim of providing a cost-effective, efficient quality service that meets the needs of the patient while fully complying with best practice standards and Trust policies in line with Patient safety and Prevention.
Main duties of the job
1. Participate in the prevention and treatment of fractures within a patient group.
2. Provide health education amongst the patient group.
3. Promote good bone health within the clinical area and advise the patient, family, and others on health promotion issues.
4. Attend daily trauma meetings, involving the wider MDT, and actively participate in the discussion and management of patients within the Fracture Liaison service.
5. Identify patients from within the data systems of patients that have suffered a fracture that meet the criteria to be referred to the Fracture Liaison service.
6. Complete Bone Health assessments.
7. Make appropriate referrals to other health care professionals and outside agencies when required.
8. Refer patients for further investigations and other appropriate services where applicable.
9. Liaise with other services to provide a patient-centered service/collaborate with nursing and medical colleagues to achieve the best care for patients.
10. Provide outpatient nurse-led telephone and face-to-face clinics and facilitate a telephone advice service.
11. Participate and complete patient follow-up and reviews.
12. Provide drug education and assess concordance of medicines and refer if required back to the relevant service.
13. Support service lead in relation to service data to inform current service delivery and future service developments.
14. Input and update patient data onto the National Database.
15. Participate in the development of the FLS at a local, regional, and national level.
