The post holder will support the Head of Department and their team to ensure that their workstream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery: The post holder will work autonomously to support and co-ordinate, where necessary, the case management of individuals with highly complex health and social care needs. The post holder will be working to support, individuals and their families/carers in accessing the most appropriate care to meet their needs. Responsible for providing day to day case management expertise for CHC eligible patients presenting with highly complex needs and circumstances including monitoring of high cost care packages and 1 - 1 provision. Define current processes, facilitate discussions and advise colleagues regarding all matters concerning safeguarding, Court of Protection, mental capacity, spinal/brain injury pathways and best interest decisions. Ensure the case management is delivered with the patient at the heart of the process and that case management issues are dealt with in line with best practice in a timely manner to achieve best outcomes for the individual. Making decisions regarding complex CHC case management issues in line with the National Framework guidance and with support of senior colleagues where needed. The post holder will be responsible for overseeing the transfer of domiciliary packages to a Personal Healthcare Budgets and writing and reviewing all Support Plans and risk assessments.
