Clinical Practice 1. Provide a patient focused clinical service to critical care 2. Demonstrate and apply the principals of effective medicines optimisation as part of day to day activities 3. To identify and address any medication safety concerns within their critical care escalating when needed to Lead Critical Care Pharmacist where necessary 4. Effective contribution to the MDT team on critical care including ward rounds 5. Make appropriate referrals as part of effective medicines optimisation demonstrating excellent clinical reasoning and judgement 6. To ensure effective clinical handover to other members of the MDT in supporting safe patient care. 7. To support and develop the clinical pharmacy service to Critical Care Leadership 1. To act as a role model for all pharmacy staff and specifically for junior pharmacists and pre-registration pharmacists 2. To promote effective clinical pharmacy services as part of the MDT/service to critical care 3. To review practices and make suggestions for improvements in services. Implement these changes in practice. 4. To promote, develop and share best practice. 5. To implement agreed clinical governance initiatives. 6. To develop clinical pharmacy services in line with local and national objectives. 7. To work with collaboratively with the whole healthcare team to deliver high quality services. Management of service 1. To provide clinical pharmacy in accordance with Trust procedures and priorities set by the highly specialist pharmacist for the area of practice or the Lead Clinical Services Pharmacist. 2. To identify and manage risks in relation to medicines use according to policy/protocol. 3. To delegate duties / responsibilities appropriately to junior staff. Identify service risks and manage or escalate the risks as appropriate 4. To provide evaluated pharmaceutical and financial information in accordance with the needs of critical care directorate 5. To identify changes in prescribing practices that may impact on the drugs budget and propose appropriate actions thereafter 6. To implement and monitor the recommendations of the EKHUFT drug formulary and other Trust clinical policies related to drug usage. 7. To participate in CQUIN and or QIPP related projects as appropriate Evaluation of Service 1. To monitor pharmacy services in order to ensure safe, clinically effective and cost efficient use of medicines. 2. To participate in pharmacy contribution audits. 3. To undertake and propose clinical and service audits as needed. 4. To be responsible developing effective action planning as part of the cycle of audit 5. To undertake medication assessments / evaluations as required. Research and Service Development 1. To demonstrate a critical approach towards pharmacy practice 2. To review research evidence and integrate into practice, sharing this knowledge as needed both within and without the trust. 3. To actively seek to improve the ward and clinical services provided. 4. To undertake practice research in accordance with the pharmacy research agenda. 5. Education and Training 1. To develop the necessary knowledge, skills and behaviours to become a highly specialist pharmacist in critical care 2. To be a practice supervisor for Foundation Pharmacists and Pre registration Pharmacists as appropriate. To provide effective supervision of junior staff and provide effective feedback to them on their performance 3. To contribute to training of other healthcare professionals including direct delivery of teaching as needed. 4. To participate in in-service clinical pharmacy teaching and multi-disciplinary teaching as appropriate.
