We have an opportunity for band 5 nurses to work within the Children's Community Nursing Team.
As a children's community nurse your responsibilities would include caring for children with Complex Care Needs, including life limiting conditions along with children receiving continuing care within their own homes. This role also includes being involved with acutely ill children with a step up or step down approach to nursing support. This is an ideal opportunity to be part of our innovative virtual ward pilot due to commence in January. Here at TRFT we strive to advance community nursing practice.
Our team provides nursing care to children and young people aged 0-19 years with long term conditions, acute and complex health needs, including long term ventilation.
You must be a highly motivated and effective team member who is committed to providing high quality care in community settings.

Main duties of the job
Work in conjunction and co-ordination with the allocated band 6 community nurse to complete assessments and care in line with the requirements of the virtual ward, working within pre-determined criteria for re referral and/or escalation to tertiary care settings to ensure safety.
Undertake individual health assessments, implement and evaluate the care of children with complex, acute and long term health needs and those who require end of life care in line with evidence based care, organisational, local and national policy and best practice guidelines.
For example, enteral feeding, pain management, bowel and bladder care, all personal cares, medication management, tracheostomy care, care of ventilated children.
Giving intravenous antibiotics, medications and wound care.
