Job summary Chelmsford City Health PCN Limited has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join our primary care network. This role is offered on a full-time basis, for 37.5 hours per week, however part-time working will be considered. Main duties of the job To support the effective delivery of patient care as part of the PCNs medicines management team and alongside the wider surgery teams. The post holder will be: A senior clinical pharmacist who will take responsibility to proactively manage patients with complex, long-term conditions such as hypertension, diabetes and COPD, ensuring that medication is optimised, titrated, monitored and complied with; Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice/PCN; Providing clinical advice and expertise on the repeat prescribing re-authorisation process to surgery staff by supporting repeat prescription systems, dealing with acute script requests and medication reviews; Carrying out and provide clinical leadership on medicines optimisation and quality improvement; Delivering on and manage relevant aspects of the PCNs Directed Enhanced Services, Quality Outcomes Framework and Investment and Impact tasks; Acting as a key contact, for all medication related queries; Liaising with other providers i.e., secondary care, community pharmacy and care/nursing homes as necessary to ensure safe transfers of care; Supporting improvements in clinical care, through practice-based audits and implementing change; Supporting the PCN clinical directors, as required.
