Home first is the ideal outcome for many patients leaving hospital, but a temporary stay in a setting that is a homely environment that can provide 24-hour care is sometimes required. The rehab and recovery beds do this, enabling early discharge from hospital and reducing hospital admissions. The rehab and recovery hub service delivers complex care whilst empowering patients as part of a wider multi-disciplinary team. When you join us, we will work with you to develop your own personal developmental plan that supports your learning style. We provide training, clinical supervision and access to a peer support network, we will enable you to develop your clinical and leadership skills.
The rehab and recovery hub service works on a shift pattern and provides a 7 day a week service. Our MDTs will deliver daily evidence-based therapeutic interventions, specifically designed to support people reach their maximum dependency state, using therapy outcome measures to demonstrate our success in achieving this. Our ethos is that the rehabilitation, recovery and reablement of service users is everybody's business.
Main duties of the job
Working in Community rehabilitation suits people who can work independently and within the wider multi-disciplinary team. You will be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care. You will have excellent communication skills and maintain a high standard of clinical work. You will ensure compliance with our policies and procedures, and you will partnership work with all members of the multi-disciplinary team and wider services.
