Job summary An exciting opportunity has arisen to manage The Urgent Community Response Team (UCRT) based here in South East Essex. The UCRT respond to the crisis needs of patients in their own homes, including nursing and residential care homes, to avoid admission to hospital or new 24-hour care. The service has access to senior nurse assessors, independent non-medical prescribers, occupational therapists and healthcare support workers to support patients to remain in their usual place of residence during an acute episode of illness or social care crisis. The post holder will be expected to be involved in the long term development and management of the Frailty Virtual Ward (FVW) as part of the virtual hospital. Main duties of the job The post holder will provide operational management responsibility for these areas providing effective leadership and hold a clinical leadership responsibility for the standards of clinical practice and services. This will include effective management of the multi-disciplinary team and financial budgets.
