An opportunity for a Senior Mental Health Practitioner has become available to join our Urgent Care Pathway. The rotational post will cover our CAMHS Crisis Resolution and Home Treatment Team and our Place of Safety (POS). The team working with Young adults under 18 who are within our urgent care pathway You will spend a defined period of time working in the CAMHS Crisis Resolution and Home Treatment Team and then moving on to the POS team. This is an excellent opportunity to hone your assessment and risk management skills, with great opportunity for continued professional development and learning. The post will involve completing holistic mental health assessments, signposting and liaison, specialist clinical input to children and young people with physical health and co morbid mental health issues (e.g. eating disorders), working with parents schools and education colleagues. Please contact us if you would like to hear more about this exciting opportunity Main duties of the job Completing assessments of young people's mental health and developing an individual plan of care Developing and implementing robust risk management plans Ensuring the physical wellbeing of the young person Representing the service at professional's meetings and relevant stakeholder events Liaison with families and carers Completing and holding oversight of mental health act processes and documentation You will be part of a robust multidisciplinary team (MDT) and attend regular clinical caseload reviews, as well as completing any associated tasks to ensure safe care and/or discharge planning To be a point of contact for young people and families as crisis arise. You will be expected to be able to employ effective de-escalation skills and to ensure that the young person is referred or signposted to the most appropriate level of care.
