Job summary SAPA 5 Primary Care Network are looking for 2 Care Coordinators to join our Personalised Care Team. Care Co-ordinators focus on providing personalised care based on local needs and health priorities. They work as part of a team to deliver well-coordinated care, especially to vulnerable patients or those with complex health issues. This job requires collaboration with GP practices, the Local Authority and other organisations to ensure patients get the best care at that time. This is a very patient facing role designed to support those in the local community. Main duties of the job Facilitate and ensure the effective delivery of patient-centred, personalised health and social care plans for patients, monitoring progress and reporting outcomes, contributing to patient reviews and care planning within appropriate time frames and clinical skills Explain the management of patient pathways to clinical staff, liaising between services and service users Work closely with all relevant care agenciesto ensure a coordinated patient care plan, without requiring a further referral from the GP To provide advice, guidance and practical support to individuals and their families/carers to ensure they are accessing the right care, at the right time, both clinically and socially Ensure that a proper handover of care between different settings has taken place Collect data on patients/carers for recognised outcome measure and document for service interpretation.
