Main duties of the job
Our teams offer support and intervention to people aged 18 plus, who live with physical disabilities, chronic and long-term conditions, autism and sensory impairment. We work in a supportive environment and are committed to professional and career development, providing monthly supervision and offering flexible working arrangements.
As a crucial member of our team, you'll collaborate with individuals, families, and caregivers, conduct Care Act 2014 assessments, evaluate needs, and manage both short-term and long-term cases. You will advance safeguarding inquiries, handle Court of Protection matters, and maintain close collaboration with healthcare and multidisciplinary professionals.

Job responsibilities
1. Undertake complex person-centred assessments (including assessment of mental capacity where appropriate) with customers and plan for the provision of support and/or services to assist them in meeting their outcomes and living as independently as possible.
2. Support carers to continue in their role in complex situations.
3. Ensure accurate and timely recording of all information and activity onto the social care records database in accordance with work targets set by the Team Manager and supervisor.
4. Gather sufficient information about customers to accurately assess their needs against eligibility criteria.
5. Contribute to local knowledge about universally available services and signpost/advocate for customers accordingly.
6. Apply advanced skills and knowledge in assessment, support planning, and risk management for individuals.
7. Practice in line with legal requirements, demonstrating a detailed understanding of the Care Act, Mental Capacity Act, and Continuing Health Care guidance.
8. Undertake Best Interests decision-making, including convening and chairing Best Interests meetings to fulfill legal requirements of the Mental Capacity Act 2005.
9. Advise, guide, and support colleagues to meet legal, procedural, and practice requirements.
10. Identify where adults may be at risk and act in accordance with the Department's safeguarding policies and procedures.
Person Specification
Qualifications
* Qualified Social Worker with evidence of post-qualifying learning and development.
* Social Work England registered.
* Proven track record of strengths-based social work with adults, including the safeguarding of adults.
* Extensive knowledge of health and social care and other relevant legislation, policies, and guidance.
* Evidenced experience of professional and personal development following achievement of professional qualification, likely to be a minimum of 2 years post-qualification.
* Evidence of giving informal supervision.
* Well-developed interpersonal skills and ability to effectively communicate with people in a variety of ways and levels.
* Ability to utilize a range of Microsoft and electronic social care record software.
* A self-starter, well-organized person who is passionate about delivering high-quality customer-centric services.
* Ability to work with others in a developmental way.
* Must be able to write clear, complex, and accurate reports.
* Fluent in spoken and written English.
* Postgraduate qualification in a relevant area, e.g. Practice Educator or Best Interests Assessor or Approved Mental Health Professional.
* Experience of giving formal supervision and appraisal.
£42,003 to £44,084 a year (Inclusive of 12.5% market supplement)
