Who You Are You are a compassionate and knowledgeable social worker with expertise in working with older adults, particularly those facing age-related frailty and mental health issues such as dementia. You have a strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, and DoLS. Your experience equips you to handle a busy caseload, managing multiple referrals effectively. As a care driver, you adapt well to an agile working environment and are willing to travel to out-of-borough hospitals as required. What the Job Involves This role is with the Later Life service in St Helens, working with individuals aged 67 and older. You will be part of a large team consisting of 15 social workers and 15 assessment and review workers. Your primary responsibility is to conduct hospital discharges for out-of-borough hospitals. This involves completing Care Act assessments, Mental Capacity assessments, best interest assessments, and attending relevant case conferences. You will devise care plans and review services based on individual needs. Regular supervision and robust support structures are in place to aid you. Work hours are Monday to Thursday from 08:45 to 17:15 and Friday from 08:45 to 17:00, with some flexibility offered, though compressed hours are not available.
