An exciting opportunity has arisen for an experienced RNLD to work as part of The Community Learning Disabilities team in Coventry covering the Coventry and Rugby areas.
The role involves close collaborative working with multi-disciplinary colleagues in the community setting, managing own caseload, supervising others, completing assessments, raising awareness of the health needs of adults who have a learning disability, identifying unmet health needs, and providing specialist nursing advice.
The successful candidate will need to be proactive, self-motivated, and have experience of autonomous working, whilst also working under the direction of the community team manager. They will also be able to use current best practice/guidance/legislation to gain the best outcomes for clients and their carers, assist in improving links with primary care, provide support, information, and education to clients, carers, and providers, and deliver education to people with learning disabilities around health needs, both on a 1:1 or group basis.
Coventry and Warwickshire Partnership Trust provides a full range of expanding physical, mental health, and learning disability services to children, young adults, adults, and older adults over many sites across Coventry, Warwickshire, and Solihull. Services are provided to a population of over one million people living in Coventry and Warwickshire, and also a wider geographical area in some of our specialist services, we see on average around 5,000 patients every day.
