An exciting opportunity has arisen to support and improve the healthcare outcomes of the Borough. Delivery of these services will be from various locations and mobile units.
Successful applicants will be providing healthcare support to those residents of the Borough that cannot access Primary Care during core hours and also those who at times need the support most but, due to their personal circumstances, are not able to access mainstream services.
The role will involve all primary care aspects as delivered by mainstream primary care but in a variety of locations and potentially using varying means.
Main duties of the job
1. Advising, supporting, and educating patients by supporting and preventing disease progression and development.
2. Support all elements of disease prevention by providing smoking cessation and associated NRT therapy, long-term condition reviews advice and guidance, and implementing care pathways related to a specific disease pathway.
3. Ensuring support and guidance is requested when required from clinical colleagues, seeking advice from other practitioners for matters outside the skill base, knowledge, and responsibilities.
4. Recording patient information such as vital signs, BMI, changes in medical history, deterioration of chronic disease and putting in place actions to support this by referral or undertaking diagnosis, during and on completion of assessments where necessary.
5. Successful applicants would be expected to work with the organisation to ensure the best possible outcomes for those that need our support most.
6. Supporting non-medical prescribing during the consultation to ensure the patient receives immediate treatment if required as an outcome of the clinical session.
7. Providing accurate diagnosis and commencement or continuation of clinical pathways during the clinical session.
8. Ensuring clinical supervision is undertaken with clinical colleagues to ensure best practice is maintained and working within the guidance of the NMC code of conduct.
9. To undertake clinical audits if requested by the clinical lead.
About us
We are a Primary Care provider within the Borough of Wigan supporting general Practice delivering Enhanced and Extended Access services during evenings, weekends, and Bank Holidays.
Our Bank ANPs support patients that cannot access Primary Care services during regular core hours.
Job responsibilities

4. Reporting to the clinical lead information from professional and patient contact to make sure care is consistent and complete.
5. Referring patients to external agencies where necessary.

* NMC registration

* Masters degree or working toward
* Advanced Nurse Practitioner certificate
* Full UK driving licence
