Main duties of the job
We are looking for a team-oriented and motivated individual who is keen to develop a patient-facing role in our practice. The role may include responsibility for specific areas of clinical care and undertaking medication reviews to proactively manage patients with complex poly-pharmacy and support our prescription team regarding prescription and medication queries. They will be actively involved in prescribing audits, supporting the practice to achieve prescribing incentive scheme targets and reducing workload on our General Practitioners.
We are part of a Federation that has a Clinical Pharmacist in post, who could assist with Pharmacist mentoring should it be required.

Salary: £46,148 to £60,504 a year. Salary negotiable depending on experience.
