Job summary There is an exciting opportunity to join the team on ward 109. We are advertising for a successful candidate to be part of our team in a position of a band 3 discharge facilitator. Ward 109 has a team of expert nurses, health cares and multidisciplinary teams who play a key part in caring for our upper and lower gastro- intestinal patients. Main duties of the job The successful applicant will be essential in supporting the nurse in charge, nurses and the wider MDT with the safe care whilst our service users are inpatients and to facilitate in safe, effective and timely discharge. The role will include a requirement to complete a varied range of documents to facilitate and support with simple and complex discharges and to have excellent communication skills with the team, patients and their relatives. We aim to give a seamless service in our team from point of admission through to discharge and create smooth flow of patients within our team.
