The Role As a Social Care Practitioner, you will be responsible for: Working directly with individuals to assess their social care needs and provide appropriate support and interventions. Conducting assessments and reviews of care packages, ensuring that individuals receive the necessary care and support to maintain their independence. Collaborating with other professionals and agencies to ensure holistic care delivery, including health services, housing providers, and voluntary organisations. Providing advice, guidance, and advocacy to individuals and their families, empowering them to make informed choices about their care. Maintaining accurate and up-to-date records of assessments, interventions, and progress, in accordance with organisational policies and procedures. The ideal candidate will possess the following skills and attributes: A relevant social care qualification Experience working in a social care setting, ideally within an adult social care environment. Sound knowledge of relevant legislation, standards, and frameworks governing social care practice. Excellent assessment and intervention skills, with the ability to work collaboratively with individuals and multidisciplinary teams. Strong communication and interpersonal skills, demonstrating empathy and understanding towards service users and their families. Effective organisational and time management abilities, with the capacity to manage a diverse caseload. A positive and solution-focused approach, demonstrating resilience in the face of challenges.
