The Home First Team ethos is to focus on supporting patient’s home earlier from hospital with an individualised package of care to meet their needs. The purpose of this post is to provide support to the Home First Team in the development, implementation and co-ordination of procedures and caseloads. Main duties of the job You will have responsibility for facilitating and support complex discharges and as a proactive leader using effective communication skills to be actively involved in changing plans and goals according to evaluation and review. You will actively and autonomously involve yourself in supporting the Home First strategy to facilitate the highest quality of patient care. The geographical area of cover is across the County of Wrexham and Flintshire to support service need. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
