The team is led by an Occupational Therapist and has found that the VdT Model of Creative Ability works very well in this specialist area. Currently, the team provides a community-based service to people with complex psychosis in the Ipswich area. You'll be working alongside a close-knit multi-disciplinary team of a Peer Support worker, Nurses, a Matron, Occupational Therapist, Clinical Psychologist, and an administrator. We work closely with the community mental health teams, social care, carers, and supported accommodation providers.
You will be supported to attend any profession-specific training and learning events to support your ongoing CPD. We are looking for someone who is keen to contribute to innovative thinking and service development.

Main duties of the job
We are looking to welcome an experienced Mental Health Practitioner, specifically an OT. You will be given full support and a comprehensive induction. Training is available in the VdT Model of Creative Ability.
We are looking for someone who is passionate about intervention. Currently, the team operates as an Intervention service, with care coordination remaining with the CMHT. This model may change in the future to a case holding team. If it does, the model will be smaller caseloads (approx 15) and a team-based approach to cases to keep rehab as the focus.
Main duties of the job
As a Community Mental Health Practitioner you will be responsible for:
1. Leading on a caseload of patients with complex psychosis
2. Assessing functioning, risk and leading specialist interventions
3. Using Dialog where possible and Recovery Care Planning with service users and carers.
4. Managing your own diary and planning your day.
5. Attending MDT meetings and wards.
6. Communicate effectively through notes on Lorenzo and letters to Service Users, GPs and other involved parties.
7. Ability to travel independently is essential for this role.

We will expect the post holder to work collaboratively and flexibly with others to undertake assessments, develop and share psychological formulations and develop treatment plans.
You will be responsible for managing a caseload, assessing and planning treatment for your patients. Working with the patient to plan care and optimise treatment plans. Feeding back to the MDT and working within the team for the best outcome for the patient.
