The post holder will ensure all care is carried out in accordance with Pennine Care Mental Health policies and procedures, the Mental Health Act and consideration for the Community Care Act via the Local Authority referral process.
The post holder will be responsible for the identification and delivery of developments across Mental Health services which increase the capacity of universal services to meet the needs of eligible clients with co-morbidity of mental health and learning disabilities.
The post holder will provide expert advice and consultation to both mental health and learning disability services, including key partnership meetings.
Within their field of expertise, the post holder will provide leadership in developing locality plans responding to national, regional and local drivers for change.
Lead thorough research in the field and identify knowledge deficits in relation to Mental Health / Learning Disabilities, plan and deliver evaluation of learning for all staff.
To carry a small caseload to undertake CPA role of Care Coordinator on a short-term basis until allocation of a Care Co-ordinator from the specialist team, i.e., Mental Health or LD (with this duty not continuing longer term).
Main duties of the job
* To act as clinical lead in the borough for the provision of care for clients with Mental Health / Learning Disabilities.
* To act as a resource to the wider Living Well MDT team. To participate in and promote expertise and education to the wider service.
* To act as a source of specialist expertise to Consultant Psychiatrists, GP’s, Inpatient Services and Community Teams in the management and clinical care of people with mental health problems and Learning Disabilities.
* To lead on building and maintaining effective working relationships with Learning Disability Service Health and Social Care Service, including agencies in the field outside the statutory services.
* To promote effective working relationships and develop protocols and policy in relation to Mental Health and Learning Disabilities Services and to develop Care Pathways.
* To co-ordinate and / or participate in joint assessments of clients with mental health problems and a Learning Disability to plan a programme of care to meet individual needs.
* To be proactive in the promotion of equal access to services.
* To lead in meetings and offer expert input in clinical decision making regarding client care.
* To act on a consultancy basis to other boroughs in the Trust to develop services and training for clients with a mental health problem and a Learning Disability.
* To act as management support to the CMHT in the absence of other line managers and to ensure the safety of team members.

Education & Qualifications
* A record license to practise.
* Evidence of recent post registration training and development at Master’s level.
* A relevant teaching qualification.
Experience
* Substantial experience of relevant post registration nursing practice.
* Working without direct supervision in the management of a complex caseload.
* Experience of working in a multi-disciplinary team setting.
* Working with people with acute mental health problems.
* Working with people with a Learning Disability.
* Experience of working with people with complex difficulties, learning disabilities and co-morbidity.
* Relevant counselling experience.
Knowledge
* A comprehensive understanding of CPA and Care Management requirements.
* Up-to-date knowledge in relation to National Learning Disability Agenda, Transforming Care and STOMP.
Skills and Abilities
* Good interpersonal skills.
* Effective communication skills written, oral and non-verbal.
* Evidence of ability to work effectively in multi-disciplinary teams.
