Job summary
An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced RSCNs/RGN with appropriate experience who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Care Framework. You will undertake comprehensive person-centred assessments so Children & Young Peoples' Continuing Care eligibility can be determined.
Roles involve assessing patients for Children & Young Peoples Continuing Care, holding a caseload, and reviewing patients, as necessary.
We also haveexciting opportunities for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.
Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.

Main duties of the job
CHC Senior Practitioner - Childrens Nurse
There are two locality place bases with current vacancies - Bury ( hrs) and Stockport (30 hrs)
The successful post holders will have the opportunity to work within a successful, progressive Integrated Care organisation and make a positive contribution to the lives of people in Greater Manchester.
The post holders will play a key role in supporting the All Age Continuing Health Care Service across Greater Manchester.
Working within the remit of the National Framework for Continuing Healthcare, the post holders will deliver an effective and competent level of service, consistently delivering a client-focused service.
The post holders will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.
CHC Specialist Practitioner (MH Nurse/LD Nurse)
There are two locality place bases with current vacancies - Tameside and Wigan.
Tameside - 1 x Full time (
Wigan - 1 x Whole Time Equivalent (WTE)

Coordinating the process of determination of eligibility for NHS funded Continuing Care within the parameters of the National Framework for Children and Young Peoples Continuing Care (2016)
Deputise for Lead Nurse in issues around Continuing Healthcare, NHS Funded Care and Complex care.
Represent NHS GM Integrated Care at multi agency meetings. Liaise with other professionals, providers and key stakeholders where required.
Liaise with providers over concerns raised at review and instigate safeguarding procedures where necessary.
Liaise with patients families, and carers with regard to National Framework for Children and Young Peoples Continuing Care and review process.
Manage a caseload of patients who have been assessed as eligible for NHS Continuing Care or a jointly funded package of individually commissioned support.

Qualifications & Proefessional Registration
Essential
1. Registered Nurse with current NMC registration
2. Diploma in Health Education, or BSC (Hons) Degree in Nursing or related subjects
Knowledge & Experience
Essential
3. Expert knowledge/understanding of current national and local policy/Framework Guidance relating to NHS Continuing Care
4. Expert knowledge/understanding of complex case management and the delivery of highly specialised care packages for Children and Young People with complex and additional health needs
5. Experience of working as a Registered Nurse with patients with a Learning Disability or Autism.
6. Experience of clinical supervision and reflective practice discussions making requires consideration of a range of options.
7. The ability to lead multi professional MDT's
8. Working knowledge of the support needs of Learning Disability individuals and associated policy and legislation
9. In-depth working knowledge of the SEND agenda, including contributing to Education, Health and Care Plans
