You will work closely with the Mental Health Liaison Team at Worthing General Hospital and The Haven at Meadowfield Hospital to provide seamless support to those in crisis. With an exciting move to the new Integrated Care Centre in central Worthing on the horizon, this role will be at the forefront of shaping how community neighbourhood teams collaborate. The successful candidate will be instrumental in streamlining pathways between services, enhancing patient experience, and improving overall care delivery.
