The post holder will work under the supervision of the clinical team and undertake appropriate duties delegated by the team. They will work collaboratively with the team to meet the needs of patients, following policy and procedures to deliver high quality responsive care.
The primary role will be to welcome patients, check their details are correct, ascertain the main reason for the patient's attendance at the PCC so they can navigate the patient to the most appropriate service for their presenting complaint. The HCA will be provided with training on this so they are made aware of all the potential service streams the patient can be navigated to. If the patient is suitable for the Mastercall PCC, then the HCA will carry out a set of initial observations including respiratory rate, blood pressure, capillary refill, heart rate, oxygen saturations, and blood glucose monitoring. Following this, the HCA will use a set of tools to establish a priority so that the rest of the team can see patients in the order of clinical priority.
