Job responsibilities Delivery of high quality, innovative, advanced practice pharmacy clinical services in both in-patient and outpatient settings (depending on experience and areas of interest). Pharmacy clinical services, including supporting self-administration of medicines, to in-patient renal wards Medicines information enquiries and expert pharmaceutical consultations related to CKD, AKI and kidney transplantation Medicines optimisation as part of the transplant work up process, including designing individualised immunosuppression protocols Referrals for individualisation of immunosuppression medicines formulations to facilitate adherence Pharmaceutical support to our haemodialysis units and home dialysis programmes Pharmacist-led renal transplant follow-up outpatient clinics Writing and reviewing relevant policies and protocols Involvement in departmental committees: renal patient information group, infection prevention, transplant and renal MDT meetings, renal board Pharmacy-led research and quality improvement Medication delivery service of renal transplant immunosuppression, ESAs and tolvaptan Education and training to non-specialist pharmacy staff and other members of the multi-professional team Designing individual peri-natal medicines management protocols for pregnant women with kidney disease or a kidney transplant Support to the young adult renal clinics and transition services from the paediatric services The role also includes the following activities: Line managing staff, training and development of team members including the trainee pharmacists, band 8a and 7 pharmacists in renal medicine and transplantation. Contribute to core pharmacy services including weekends, bank holidays early evening shifts and on-call on a rotational basis. Maintain and develop an expert clinical practice in area of Specialty. Deliver highly specialised pharmaceutical care to patients and to support other healthcare staff in meeting risk management, financial management and clinical governance responsibilities around the use of medicines. Evaluate incidents involving medicines, share the root cause and learning with relevant teams. And to take appropriate action to help reduce future occurrence.
