Main duties of the job The successful applicant will be responsible for coordinating and facilitating the discharges of patients identified as medically optimised for discharge. Arranging medications for discharge, arranging transport for discharge, liaising with care providers and care homes to enable a timely discharge, ensuring good use of the discharge lounge when availability allows. Excellent communication skills are essential to ensure patient and families are kept informed about discharge plans. The successful applicants will be responsible for keeping excellent contact with the patient flow matron, they will work closely with the ward domestics and the nursing staff to ensure bed spaces are cleaned and ready to accept admissions in a timely fashion. You would need to be confident, an excellent communicator a mature decision maker and work well under pressure, you will be efficient and organised with an eye for detail and commitment to improve the patient experience.
