Allied Health Care Professional (ANP, ACP, Paramedic)
We are looking to welcome a caring professional - ANP, ACP, Paramedic or Pharmacist with Minor Illness skills into our practice, to work full/part-time. The main responsibilities of the role include delivering a high standard of patient care, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base. Assessing, diagnosing and treating patients' conditions, initiating direct referrals as appropriate.
Main Duties of the Job
Ideally, you will be an independent prescriber, but this is not essential for the right candidate. You will be able to telephone triage, manage e-consults, and work as part of an acute hub as required. As a vital part of our multidisciplinary team, you will support and care for a diverse patient group across our four-site practice. About Us
St Bartholomew & Hollow Way has a large team of 60, including 6 GP partners, salaried GPs, pharmacists, and Allied Healthcare staff, supported by an invaluable admin and reception team.
Our main surgery is at St Bartholomew's, on Manzil Way. We also have three branch surgeries, one of which cares for the students and staff of Oxford Brookes University, and the others are at Lake Street and Hollow Way Medical Centre.
We are a training practice, supporting trainee GPs, medical students, pre-reg nurses, and pharmacists. We work hard, support each other as a team, and encourage everyone to achieve their work-life balance.

Job Responsibilities
Job Purpose
The post holder will consult with patients presenting for minor illness assessments face-to-face, over the phone, and through electronic means. The focus of the role is both the delivery of evidence-based practice for patients presenting within the Acute Hub with both long-term conditions and/or minor illness and will promote and support the provision of preventative health care to the practice population. As an autonomous practitioner, you are responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively within and be supported by the general practice team, especially GPs, to meet the needs of patients and support the delivery of policy and procedures.
Key Responsibilities
1. To act as an autonomous practitioner providing clinical assessment, diagnosis (where possible), and treatment (where appropriate) for patients (Adults & Paediatrics) presenting with minor illness and advising people in respect of their continuing medical and nursing needs.
2. To clinically triage and manage e-consultations daily as required through an electronic system.
3. Prescribe, supply and/or administer medication and treatments according to agreed protocols, within scope of competence.
4. To maintain accurate contemporaneous computerised clinical records of every contact, including history, examination, diagnosis, plan of care, advice given, treatment, and recommended follow-up.
5. Assess, plan, implement and evaluate individual treatment plans for patients with a known long-term condition.
6. To be flexible and responsive both to the clinical needs of patients and operational needs of the service.
7. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
8. Undertake medication reviews in line with practice protocol as identified, face-to-face with the patient.
9. Initiate tests and follow through results by access to path links.
10. Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
11. Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
12. Work with patients to support concordance to prescribed treatments.
13. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects, and interactions.
14. Support patients to adopt health promotion strategies that promote patients to live healthily, and apply principles of self-care.
15. Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance.
16. To be aware of and work within the performance framework for Care Quality Commission and the National Quality Requirements.
17. Continue to develop, expand and role model own personal clinical expertise as an autonomous/independent practitioner.
18. To contribute to the development of clinical services within the practice.
19. To contribute to the development of the practice as a learning organization, using your advanced skills to lead, train, and support junior staff.
20. To contribute to the work of our practice team, undertaking clinical supervision and training of colleagues.
Person Specification
Qualifications
* NMC registered or equivalent.
* Desirable but not essential: Prescriber.
* Experienced in telephone triage.
* Experienced in dealing with e-consults.
