We currently have a vacancy for a Community Nurse, working with the Launceston and Tamar Valley Community Nursing Teams and to work flexibly across the East locality when required. Situated in beautiful East Cornwall, we are busy, dynamic, and forward-thinking.
We are seeking to recruit a Community Nurse with a commitment to provide compassionate, holistic patient care, working flexibly across the area. You must possess excellent communication and people skills; have the ability to work autonomously with little direct supervision, whilst being a committed member of our supportive team.
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
Main Duties of the Job
1. Responsible for the assessment, planning, implementation, and evaluation of care, without direct supervision, to patients, carers, and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.
2. Work in partnership with patients and carers to offer support and advice and promote informed choice.
3. Support the role of the Community Nurse Team leader.
4. Work autonomously, providing effective, evidence-based care in line with competency and training record and organizational policies, procedures, and guidelines.
5. Support all locality teams by working flexibly, moving to work with the team in most need as service provision demands.

Qualifications
* Current Professional Registration - NMC
* Evidence of recent relevant personal professional development
* Hold a post-registration diploma or be working towards a degree
Skills
* The ability to provide skilled, evidence-based nursing care to a wide range of patients
* Excellent communication and interpersonal skills
* Ability to deal with patients and carers who are under stress
