An opportunity has arisen for a suitably qualified enthusiastic and motivated nurse to join our friendly and supportive Community Team for People with a Learning Disability (CTLD) in Hywel Dda University Health Board, based in Carmarthen.
This post comes during a period of Service Transformation in response to the changing needs of people with a learning disability and the national agenda for improvement. If successful, you will play a vital part within this exciting period of change, by contributing to the design of a new service model providing flexible & responsive services.
As part of this Service Improvement Programme, we are keen to support nurses who are willing to share new ideas and suggest improvements. We have close links with Improvement Cymru and have access to dedicated in house project management support.
Main duties of the job
You will be expected to work proactively and creatively, using your skills and experience as part of a multidisciplinary team to deliver high quality, evidence based nursing care to adults and young people with a learning disability in partnership with them and their families/carers. You will have the opportunity to work closely with other colleagues in the multi-disciplinary team, and will provide specialist advice, support and training to colleagues, families and carers in addition to the people on your caseload.
The ability to speak Welsh is desirable for this post English and/or Welsh speakers are equally welcome to apply.

* Registered Nurse RNLD with a current (or pending) NMC registration
* Good understanding of current policy and practice in relation to people with Learning Disabilities.
