1. Qualification: To hold or be willing to undertake and achieve a Level 3 Health and Social Care Diploma (QCF)
2. DBS (Disclosure and Barring Service) Clearance: This post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check.
3. Evidence of conduct in previous employment concerned with the provision of services relating to Health or social care or Children or vulnerable adults.
4. Proof of identity including a recent photograph.
5. Full Employment history.
The role holder will provide support as part of a short term reablement service which coaches and encourages customers to learn or re-learn daily living skills, enabling them to confidently and independently live in their own homes and take part in the community.
To support the customer to achieve their goals, promote independence, health and well-being throughout the ‘make safe and stabilise’ and/or reablement support period.
Provide person-centred, high quality support to customers in their own home or in other appropriate locations, to maintain and develop daily living skills that will enable full participation in a range of activities which support customers to access appropriate community services.
Provide practical, personal and intimate care whilst maintaining high levels of hygiene and health and safety.
Work with customers in goal setting, as detailed in the support plan to enable the customer to maximise their independence and rehabilitation through reablement programmes and emotional support.
Establish customer needs and think innovatively to deliver solutions utilising local community groups/resources to support the customer to achieve their goals.
Complete all documentation and correspondence accurately and to a high standard in line with procedures and within agreed timescales.
Work in line with National Care Standards, Care Quality Commission and City Council policies and procedures, incorporating changing legislation and best practice.
Proactively participate in new initiatives and future changes in service delivery.
Demonstrate a clear understanding of care procedures to provide high quality care and support to service users.
Establish service user needs and think innovatively to deliver solutions utilising local resources.
Contribute effectively and innovatively with the creation and implementation of assessments, plans and risk management.
Provide personal, practical and if required intimate care whilst maintaining high levels of hygiene and health and safety.
Ensure that records are completed and maintained with sufficient information and accuracy in line with agreed service standards and timescales.
Personal commitment to continuous self-development and service improvement.
Through personal example, ensure diversity is positively valued.
