We are looking for a Clinical Pharmacist to work across Cordwallis Rd and Woodlands Park GP Surgeries. The Clinical Pharmacist will act within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role, they will be supported by a Senior Clinician and Line Manager who will develop, manage, and mentor them.
Required: 30 hrs per week.

Main duties of the job
* See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g., COPD, asthma).
* Undertake structured medication reviews with patients and produce recommendations for agreement on prescribing changes and monitoring.
* Provide patient-facing clinics for those with questions, queries, and concerns about their medicines in practices.
* Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
* Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists.
* Signpost to other services and/or healthcare professionals where appropriate, while working within scope of practice and limits of competency.
* Manage repeat prescription requests, resolving queries where possible within scope of practice.
* Maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
* Provide independent information to patients regarding medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate.
* The post holder will be supported to develop their role to become a non-medical prescriber.
* Work with practices to ensure full compliance with Care Quality Commission standards for safe and effective care.

Qualifications
* Mandatory registration with General Pharmaceutical Council; Masters degree in pharmacy (MPharm) or equivalent.
* It would be ideal if completed both or either the PCN CPPE or IP course.
* Membership of the Royal Pharmaceutical Society; A member of or working towards Faculty membership of the Royal Pharmaceutical Society; Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience; Independent prescriber or working towards/intent of gaining independent prescribing qualification.
Experience
* Minimum of 2 years post qualification experience.
* Experience of working in primary care.
