Job Summary:
We are looking for an experienced and enthusiastic Care Coordinator to join our growing team. The successful candidate will be responsible for liaising with care staff and clients and ensuring that our services are delivered efficiently and compassionately. This role requires excellent organizational skills, attention to detail, and a passion for providing outstanding care.
Role and responsibilities:
* Liaise with care workers, service users, and families to ensure care plans are effectively implemented.
* Monitor and update care plans according to client needs and requirements.
* Ensure compliance with company policies and industry regulations and to carry out competencies with members of staff.
* Respond to queries and emergencies in a professional and timely manner.
* Maintain accurate records and reports in line with company and regulatory requirements.
* The successful applicant will be on call as part of their role.

Requirements:
* Previous experience in a Care Co-Ordinator role or similar within the health and social care sector.
* Strong organizational and multitasking skills.
* Excellent communication and interpersonal abilities.
* Knowledge of CIW regulations and industry standards (preferred).
* Proficiency in using care management software and Microsoft Office.
* The ability to deal with conflicts in a professional manner.
* A caring and compassionate attitude towards clients and staff.
* Ability to work under pressure and adapt to changing circumstances.
* A valid UK driving license and access to your own vehicle.
