Main Duties and Responsibilities Work in partnership with key partners and stakeholders in the delivery of the Service, in particular the local Council Adult Social Care Departments and local Provider of NHS Services and VCSE to develop the strategies, projects and delivery plans that form part of the ICB Operating Framework. Develop and maintain strong working relationships with partners which is cognisant of statutory responsibilities, budgetary restrictions and scope of practice. Ensure close multi-agency and multi-disciplinary working across a range of organisations to ensure the individual is at the heart of all plans and performance monitoring. Demonstrate a clear understanding of the commissioning arrangements and care pathways in designated service areas and maintain day to day liaison with Providers regarding the ICB requirements and keeping them informed and up-to-date about them. Provide evidence based and current clinical expertise and activity to support the delivery of Transforming Care across Kirklees Ensure a comprehensive range of high quality, responsive and efficient services are designed and delivered within allocated resources. Identifying new areas of service redesign and development in specific service areas that would improve quality, productivity and prevention including the development of scoping documents, business cases and project documentation as required. Ensuring adherence to local and national policies and keeping up-to-date with legislation which impacts on the corporate management across Kirklees particularly in respect of own area of accountability. Provide expert and skilled advice to partner organisations in the assessment, identification and delivery of care. Be responsible for the appropriate delivery of Section 117 Aftercare eligibility assessment and decision- making processes in own area, and provide oversight of funding eligibility challenges and appeals. Ensure attendance as required at the relevant clinical reviews and multi-disciplinary discussions and decisions for individuals placed in hospital settings, including Forensic Services to ensure that complex patients care packages are appropriate to need and the delivery of care is safe and effective. Support the relevant needs assessment, design and commissioning of Services to meet the needs of the Kirklees population living with mental health, Learning Disability and/ or Neurodiverse diagnoses. Responsibility for delegated contractual budgets and monitoring performance against these. Provide a line management function and HR responsibility for the Complex Care Team. Take the clinical lead on day to day operational issues relating to the programme area, providing management and leadership to the Team Leaders in the Complex Care Team, including advice and support to the Continuing Healthcare Team when required. Responsible for staff development including the setting of objectives, and agreement and supported delivery of Personal Development Plans.
