We are seeking experienced individuals to join our dynamic team as Community Service Assessors (Assessment Review Officers). If you are passionate about helping individuals regain their independence and stay safe in their own homes, this could be the perfect role for you. About the Role As a Community Service Assessor, you will play a crucial role in our Reablement Service. Your primary responsibilities will include assessing new individuals to the service, identifying the support they need to remain in their homes, and writing person-centred support plans in collaboration with them. You will also review support plans through regular visits, helping people achieve their long-term goals. This role involves lone working and collaboration with multiple agencies, working towards KPI targets to support both Herefordshire Council Adult Social Care and Wye Valley Trust. You will be required to work every other weekend and travel regularly, so access to a car is essential. Key Responsibilities Conduct assessments of individuals new to the service to identify the support needed for independent living Develop person-centred support plans and agree on the level of care and number of visits Review and update support plans during regular visits, setting long-term goals with the individual Work independently and as part of a multi-agency team to meet KPIs and support key partners Regular travel across Herefordshire (mileage paid) Experience & Qualifications Level 2 or 3 qualification in Health and Social Care (or demonstrable experience in a similar role) Minimum of 1 year’s experience in Health and/or Social Care Experience in support planning and risk assessments is essential What We Offer Competitive Salary : £28,971 per annum Paid Mileage : 45p per mile 26 Days Annual Leave (31 days after 5 years' service) Bank Holidays Paid Overtime Cycle to Work Scheme Discounted Shopping Career Progression : Opportunities for career development within Hoople Childcare Voucher Scheme Uniform and Mobile Phone Provided Discounted HALO Leisure Membership Funded Qualifications Why Work With Us?
