Job summary The post holder will operate as part of a multi-disciplinary team, providing support and guidance for patients requiring support. The post holder will undertake physical and/or health assessments, provide and evaluate a range of health treatments and interventions to meet the needs of the patient, and contribute to the wider continuous development of prison healthcare services. The post holder will demonstrate knowledge, skills and understanding of Long Term Conditions providing health promotion and advice on treatment options. Main duties of the job The role includes the assessment of complex care needs, the development, implementation and evaluation of clinical pathways, the setting of standards and management of staff. The post-holder will work in close partnership with the Head of Healthcare and wider clinical teams to ensure safe, effective implementation of best practice and robustly risk managed services, whilst simultaneously facilitating cross partnership working.
