Job Description
We are excited to be advertising for a SEND and Transitions Lead (Band 7, 0.6 WTE) to support the development and integration of Special Educational Needs and Disabilities and transition processes across Children and Family Health Devon. This role will play a key part in ensuring that children, young people, and their families receive seamless and coordinated care as they navigate transitions in health and education services.
The successful candidate will provide expert advice on SEND health components, contribute to policy development, and support multidisciplinary teams in delivering high-quality, person-centred care. Main duties of the job
1. Ensure the organisation embeds Special Education Needs and Disabilities (SEND) and transition processes and pathways, through planning and organising the delivery of appropriate training, advice and guidance to staff regarding SEND and transitions to adult services.
2. Offer expert advice in the formulation of health components of an Education Health Care Plan (EHCP) across all clinical pathways as required, being the primary point of contact for all health-related queries regarding personal health budgets and SEND.
3. Provide specialised information, advice and guidance concerning SEND and transition to adult services, utilising clinical reasoning skills and evidence-based knowledge.
4. Develop and maintain positive, collaborative working relationships with colleagues at all levels across CFHD, provider organisations Torbay and South Devon NHS Foundation Trust (TSDFT) and Devon Partnership NHS Trust (DPT), Alliance partners, service user groups and all other stakeholders.
5. Represent health at interagency committees and boards in relation to SEND/EHC and transition Planning.
6. Obtain, collate and analyse complex information to assess the performance of the organisation against objectives, ensuring performance targets are met in accordance with the commissioned services and duties under the SEND regulations.
7. Ensure information about SEND and health transitions to adult services is accessible to children, young people and their families/carers.

Children and Family Health Devon (CFHD) comprises an Alliance of NHS organisations working together to improve health outcomes for children and young people in Devon. CFHD provides integrated care and treatment for children and young people across physical and mental health, which are provided by Torbay and South Devon Foundation Trust and Devon Partnership Trust.
Person Specification
Experience
* Experience of leading and managing multidisciplinary teams.
* Experience of providing professional teaching and training for other professionals working with children/young people and families.
* Experience of using data to improve services.
* Experience of working collaboratively with service users and their families/carers.
* Experience of working collaboratively in a multi-disciplinary team and across agencies.
Qualifications
* A professional qualification at degree level or equivalent e.g. Psychological profession, Nursing, Social Work, Allied Health Profession.
* Additional specialist training and qualification to post graduate diploma level or equivalent experience in supervision and/or leadership skills.
* Professional registration/accreditation with a recognised body, e.g. NMC, HCPC, Social Work England.
Knowledge, skills and abilities
* Knowledge of relevant legislation, NHS policy and other national policy relating to children and young people.
* Must demonstrate understanding and interpretation of The Children and Families Act 2014, SEND Reforms 2014 and relevant NICE guidelines.
* Knowledge of evidence-based practice including NICE guidelines and routine outcome monitoring.
* Understanding of the quality, governance and regulatory framework within the NHS.
