Job summary An excellent opportunity has arisen to come and be part of our transformational multi-agency HOMEFirst Alliance. HomeFirst is an exciting initiative using multi-agency collaboration to provide enhanced, recovery-focused support in the community for those living with complex mentalhealth difficulties, that often lead to admission to acute hospital. The approach brings together multi-agency working and good quality care planning to provide enhanced community support. Please note this role is 12 months, fixed term. We will consider applicants with professional registration in Occupational Therapy, Social work and Nursing. The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role. Main duties of the job The post-holder will have a base at Trust HQ in Leatherhead but will be working alongside Community Mental Health Recovery Services acrossSurrey, with virtual working included.1. To provide high quality and evidence based professional assessment and interventions (group and individual) to people who access services and their carers/supporters/family members, in line with the HOMEFirst model and with support/supervision from a Lead occupational therapist. 2. To communicate effectively with people who access services, carers, the multi-disciplinary team and when appropriate outside agencies. 3. To contribute to the generic functioning of the team. 4. To participate in service development and audits.
