We are looking for enthusiastic and self-motivated individuals to work as part of the Community Ward Team in Halstead.
Halstead ward is a 20 bedded ward providing a service supporting Intermediate Care (rehabilitation) to residents in the Mid and South Essex area. The ward is nurse-led, supported by Medical Practitioners, and provides a progressive service which promotes multidisciplinary working. The philosophy is one that encourages staff participation at all levels and identifies opportunities for service and career development.
Main duties of the job
The successful applicant will be required to integrate flexibly across the ward with a particular focus on communication with the MDT to support a team approach to care. A key requirement of the role will be to deliver safe, high-quality care that meets the needs of increasingly complex patients in the community wards. This will involve joint working between all of our inpatient units and supporting community services to manage patient pathways safely and effectively and ensure that robust discharge plans are in place that support patients to live their lives as independently as possible on discharge. There are close working relationships with medical practitioners and MDT which encourage a seamless and proactive approach to planning successful discharge.
