Develops excellent communication skills to facilitate discussion of complex, sensitive and emotive issues around cancer diagnosis, disease management, recurrence/progressive disease and prognosis and refer to other practitioners when a patients individual health outcomes are beyond own competence. Manages the care of patients with a suspected/confirmed diagnosis of a cancer, providingspecialist advice within a defined clinical area and assists in the follow-up of patients following a cancer diagnosis. Counsel, educate and empower patients and carers regarding management interventions, treatment options and care. Including explaining potential side effects of treatment and explaininginformation regarding life style adaptations e.g. Sun protection advice. Assess, potentially highly complex, patients, planning, implementing and reviewing care within the hospital environment, both in-patient and out-patient, community or other settings, liaising with external agencies as appropriate Using holistic needs assessment, develop care plans, and implementation strategies through shared decision making with patient and carer.
