The Lawns Medical Practice located in Diss, are seeking an experienced and dedicated practice nurse to join our team on a part-time basis for 32 hours per week. We are a supportive and friendly practice with a manageable workload. As a skilled practice nurse, you will be expected to be able to perform core clinical nursing duties and must be happy to engage in ongoing training to increase your skills and range of consultations. Our nurses work collaboratively with our GPs and other medical staff to ensure our services are efficient and effective to meet patient needs and give the best care we can. You will benefit from the full support of the team. This varied and interesting role will include the following responsibilities; Undertaking health assessments, administering medication and treatments. Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, i n line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Wound care. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN. Previous clinical experience. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS.
