Job summary Are you a Registered nurse looking to develop into a specialist role? A wonderful opportunity has arisen within the established Heart Failure teams within Sussex Community NHS Foundation Trust supporting the populations within Sussex as a B5/6 Development Nurse Practitioner. The post holder will be supporting the management of complex heart failure patients with a collaborative aim in keeping patients well at home and to avoid hospital admissions where possible along side the senior Nurse Specialists within the service. This role will consist of a 12-18 month Development Pathway where the successful applicant will commence in post as a B5 Development Nurse where on successful completion of L6 University Modules, Internal training and completion of Clinical Competencies will progress into the B6 role. The successful applicant will be assigned a mentor within the service and will be supported in regular supervision, clinical discussion and reflective Practice.
