This is a partnership between the University Hospital of North Midlands (UHNM), University Hospital of Derby and Burton (UHDB), and Midlands Partnership Foundation Trust (MPFT). The successful applicants will join an integrated, collaboratively focused team, employing proactive clinical practice and knowledge, to provide clinical assessment, monitoring and support, as part of our urgent care pathways.
Community Rapid Intervention Service (CRIS) and General Acute Medicine and Frailty Virtual Ward is a new and innovative Service, that has an agreed admission criteria and works in partnership withsupporting care services to deliver acute care in the patient's own home. This means we can proactively manage people living in there own home, and for those that have a sub-acute episode we can improve a patient'sexperience and outcomes. In addition, this means we have more space in ourhospitals for acutely unwell patients and reduces the pressure on urgent care.

Main duties of the job
Communicating very sensitive, complex condition related information to patients, relatives providing empathy and reassurance.
o Exchanging verbal and written information with patients, staff and carers requiring tact and diplomacy.
o Communicating patients' conditions verbally and in writing, to other members of the healthcare team and contribute to decision making processes in patient management.
o Communicating on an ongoing basis with the Deputy Senopr Operational Manager and /or the Virtual Ward Matron and/or the Consultant of the Week supporting the Virtual Ward service updating them of any changes/progress.
o Taking part in the appraisal process each year with the first appraisal taking place within the first 6 months from appointment.
o Continually updating and maintaining personal levels of clinical skills and competencies, acting as a clinical role model.
o Having a current NMC /HCPC registration Having experience of teaching and assessing.
o Having undertaken leadership training.
o Having a minimum of 3 years post registration experience to include 1 year experience of a leadership position.
o Having the ability to lead change and lead new ways of working.
o Undertaking physical assessment.
o Independent prescribing.
Dealing with highly complex facts or situations, requiring analysis, interpretation and comparison over a range of options.

Job responsibilities
The post holder will be a Registered Healthcare Professional and hold an MSc in Advanced Clinical Practice or equivalent at MSc level to be agreed as part of the application process. They will be an independent Non-Medical Prescriber. They will practice at an advanced level demonstrating in depth knowledge and competence in all aspects of acute care within the patients home setting & environment, encompassing enhanced assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.

* Substantial post registration experience of which significant experience is within Emergency/Acute/General Medicine Care at a senior level
* Evidence of extensive expert professional/clinical knowledge in Emergency/Acute/General medical care supplemented by specialist clinical training and CPD (
* o Evidence of involvement and leadership in teaching, supervising and mentoring learners
* Evidence of working on own initiative managing time and prioritising
* Excellent Clinical reasoning/clinical decision making
* Evidence of proactive involvement in the mentorship and development of junior healthcare professionals
* Evidence of involvement in policy and practice
* Evidence of autonomous decision making within a patient's own home
* Evidence of providing acute care within a community setting and/or service
