We are seeking a motivated Occupational Therapist to join our Locality Therapy Team. The successful individual will be based at Ludlow Community Hospital. They will join an enthusiastic and supportive multi-disciplinary team in providing high quality care and rehabilitation.
The successful individual must have an interest and experience in inpatient rehabilitation and be able to work as an autonomous practitioner. The post holder's main responsibilities will be providing Occupational Therapy assessment and rehabilitation to inpatients who have a wide range of conditions and complex needs. They will be working alongside colleagues to contribute to safe and effective discharge plans, taking an active part in Red2Green approach to help reduce unnecessary delays for patients. As well as working alongside community colleagues to develop streamlined pathways of care.
We have a strong commitment to clinical supervision and Continuing Professional Development (CPD), offering a supportive and proactive environment with opportunities for supervision, training and peer support, both locally and County wide. This post will be involved in supervision of junior staff, rehabilitation technicians and students.
Main duties of the job
* To provide a high quality seamless Occupational Therapy service from Acute or Community Hospital to Community, responsive to the needs of the local population as part of a cohesive multidisciplinary team.
* To work as a key member of the multidisciplinary team (MDT) that is responsive to the needs of the local population, working flexibly to respond to the changing needs of the patient.
* To provide a key role in facilitating the seamless transition from community hospital to patient's own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.
* To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.

* Diploma/degree in Occupational Therapy
* Current registration with HCPC
* Evidence of core rotations
* Evidence of specialist post graduate training and education with older adults.

* Significant Post graduate experience in a variety of clinical settings with experience in a community setting and inpatient setting
* Experience of working within NHS or equivalent setting.
* Experience of working in multi-disciplinary / multi-agency teams.
Skills and Knowledge

* Ongoing awareness of recent developments in practice and research including NSF's/NICE guidelines.
* Understanding of clinical governance and risk management and their implications for the service.
