We have a new and exciting opportunity for an enthusiastic Dietetic Assistant Practitioner to join our friendly and dynamic team at South Tees Hospitals NHS Foundation Trust. This post will be based at our community site at Langbaurgh House, Guisborough.
Our mission in the community Dietetic team is to use a patient-centred approach to make a meaningful impact by educating, empowering and supporting. We enable people to feel seen and heard.
Working within defined scope of practice and service guidelines, key aspects of this role will include:
1. Assessing the nutritional needs of non-complex adult patients with a range of medical conditions.
2. Devising programmes of dietetic care under dietetic supervision.
3. Monitoring and evaluating the effectiveness of dietary intervention.
4. Providing regular feedback to the supervising dietitian.
Our service delivers care across 2 primary care hospitals and within community settings across the South Tees locality, therefore you will need to be able to travel independently between sites.
It is vital that you have strong and confident communication skills when liaising with patients, carers and relatives, and the ability to develop good working relationships with a wide range of professionals working across health and social care. This post also offers exciting opportunities for continued development in a well-established service.
This role is ideal for a candidate who has worked previously as a Dietetic Support Worker or an Assistant Practitioner who has had significant involvement in supporting patients with their nutrition and hydration care needs.
