Job Opportunity
Lead the care team, in the daily administration of person-centred care, maintaining the expected atmosphere and practice of care, based on the values of Cornwallis Care Services.
6.2 Ensure the person-centred care planning process on Nourish is up to date and maintained. Evaluate and develop care plans through routine auditing, ensuring the team are responding to the changing needs of individual residents and documenting appropriately.
6.3Hold overall responsibility for the correct and efficient ordering, safe administration, storage and recording of prescribed medication, ensuring the Camascope EMAR system is correctly utilised by the team.
6.4 Be responsible for undertaking observations of practice for team members; providing constructive feedback to support their continuous development. To carry out competency checks on your team, ensuring those assigned to deliver medication are suitably trained and fit to do so.
6.5Ensure prompt and open communication with families regarding the care and wellbeing of their relative/friend.
6.6Liaise and work with external agencies, GPs and other professionals ensuring the best interests of those in our care.
6.7To carry out on call duties in line with the On Call Duty Rota.
6.8To raise safeguardings and notifications as required, to ensure transparency and candour with the commissioning bodies.
6.9Completing and submitting allocated audits and reports in line with the Company’s Audit & Reporting Schedule
6.10To be part of the care team as required; directly providing personal care to residents and support in all aspects of their daily living as identified in their care plan.
6.11To support the Manager, when required, with pre-admission assessments for prospective residents off-site and facilitate their safe admission to the Home.
6.12Develop and maintain good communication and working relationships with and between colleagues.
6.13Ensure that the standards of cleanliness of the Home are maintained on a daily basis and that all infection control measures are in place.
6.14Ensure that all appropriate notifications are submitted to CQC as required.
6.15To resolve all complaints in accordance with Cornwallis Care Services Complaints and Compliments Policy, ensuring that the Home Manager and/or Staff Supervisor are informed without delay.
Essential Skills

7.4Lead and Participate in quality assurance systems as required.

7.6To undertake any other duties within the scope of this role profile, at your normal place of work or at any other of the Company’s establishments.
7.7The post holder is responsible and required to observe strict and complete confidentiality regarding information obtained during the course of his/her duties.
7.8To be responsible for your own health and safety and that of anybody else whom your acts or omissions may affect.
7.9To work in accordance of Cornwallis Care Services policies and procedures at all times.
