Care Manager - £27,040.00 to £28,000.00 per annum.

We are looking for a highly motivated, enthusiastic, and experienced Care Manager to join our Learning Disabilities division.
Hours: 40 hours per week, this will be inclusive of some weekend work.
Objective of Role:
You will be working within our learning disabilities projects based in Ipswich to guarantee the delivery of high-quality, person-centred care, through leading by example from a ‘ground up’ basis. You will assist the Registered Manager to achieve CQC compliance in all areas.

Main Duties:
* Be responsible for the day-to-day operational case management, assessment, and delivery of specialist social care interventions for adults with learning disabilities, operating within a culture of continuous service improvement and safety to ensure professional standards and best practice are embedded throughout your teams.
* Provide excellent leadership and direction to your staff teams so that they can understand, embrace and deliver the strategic aims of the service.
* Be responsible for all aspects of leadership, management and development within the service. This includes responsibility for effective communication and information sharing.
* Actively manage, supervise, and participate in the delivery of care and support, always promoting independence, inclusion, dignity, choice, and respect.
* Improve choice and quality of life by timely interventions and/or review and reassessment that maximise long-term independence and minimise ongoing support.
* Ensure service user’s choice and independence is respected through risk management involving all parties to positively promote their best interests and ensuring accurate up-to-date record keeping and production of, or adaptations to, risk assessments.
* Respond to emergencies and provide appropriate guidance and support to all staff in the event of the same.
* Be responsible for collaborative reviews and updating of all service user Care Plans, whilst being proactive in the continual monitoring of each service user’s needs liaising with all relevant parties.
* Welcome service users to be involved in the service and their plan of care, ensuring the staff team encourage their participation in meaningful activities to foster emotional and intellectual stimulation.
* Work closely with families and friends, encouraging and facilitating relationships important to our service users.
* Complete regular service audits, including staff medication competencies and accurate MAR chart production, use and completion.
* Effectively lead, motivate and support staff, assisting in a robust induction process.
Person Specification:
* Hold a relevant professional qualification such as a NVQ Level 3 in Health and Social Care or equivalent.
* Experience leading a health or social care team.
* Effective communication including written, verbal and interpersonal skills.
* Clear and thorough report writing.
* Contribution to service planning.
* Record of high performance in successfully delivering results.
* Experience taking the lead with adults at risk where there are safeguarding concerns.
* Working in partnership with internal and external stakeholders to produce positive outcomes.
* Sound, practical understanding of relevant legislation (such as the Mental Capacity Act), policies and processes necessary to deliver complex services to vulnerable adults.
