Job Title: Occupational Therapist – Community rehab and nursing home settings
Location: Tower Hamlets
Rate: £35.45 per hour

What will your responsibilities be?
You will conduct Care Act assessments for individuals discharged from hospital to step-down beds. Collaborating with a multidisciplinary team (MDT), you will support individuals in returning to their homes and communities. You will arrange necessary care and support to facilitate a smooth transition and perform Continuing Healthcare (CHC) assessments, mental capacity assessments, and functional assessments as needed.

Qualifications and Experiences:
To be successful in this role, you must have a degree in Occupational Therapy, be registered with the HCPC, have post-graduate experience. Additionally a full UK driving licence is required.
